You may be able to pay with your FSA/HSA card through our partnership with Flex. Please make sure to select Flex at checkout and follow their procedure to see if you are eligible. To use Flex, make sure you are not logged in to ShopPay during checkout.
Flex will administer a short health questionnaire to provide you a letter of medical necessity to clear the reimbursement. Once you have received this letter, it will be valid for the continuing year for all Forme purchases. Please note that health insurance is separate from HSA/FSA, and Flex’s documents will likely not cover use in that case. As always, please consult with your physician if you have any medical questions or concerns.
