Return and Exchange Policy

We will happily accept returns and exchanges for eligible products in original condition within 30 days of the order payment date with proof of purchase subject to the terms as stated below. Visit our return portal to start a return or exchange. 

*Currently, there may be backordered items. Backordered items are subject to the return policy, with the return and exchange window starting within 30 days of the order delivery date.

• Returns or exchanges must be initiated within 30 days of the order payment date. 
• Items must be in original condition, unworn, unwashed, and undamaged in original packaging with the tags still attached. We reserve the right to deny returns upon physical inspection if they do not meet the requirements.
• All returns are handled on a case-by-case basis and ultimately at Forme's discretion with physical inspection, including determining whether an item is unworn and unwashed with the intent of providing the best experience to our guests. Potential fraud or abuse of this policy, including excessive or unreasonable returns, may result in the rejection of future transactions or returns. 
• If we choose to provide a refund beyond 30 days, refunds will be applied to a Forme gift card.
• Please check out with Flex for your purchase to be eligible for HSA/FSA. They are not able to issue receipts post-purchase. 
• One exchange with free shipping is allowed per original order.
• If you do not ship using our provided return shipping labels a refund or exchange will not be issued. We do not accept returns in any other way.
• Items must be shipped within two weeks of the label shipping label issued date. 
• Refunds will be processed within two to four weeks of the returned product(s) being marked as delivered by the postal service tracking number using the exact provided shipping label(s). Do not combine items from different order numbers in one package. Route and shipping costs are non-refundable.
• The Ace Polo, Arch Booster Sock, Ankle Guard Sock, Men's Ace V-Neck, and Pro Short are final sale. Final sale products are not eligible for returns or exchanges. 
• Return shipping and processing fee at $15 per return will be deducted from the refund amount.
• Defective products must be reported within 48 hours of delivery to qualify for a return or exchange and be returned back to us.
• Orders that have already been placed are not eligible for price adjustments. A discount cannot be applied to an exchange. 
• Route Package Protection is insurance for lost, stolen, or damaged packages. Please consider using Route Protection as an insurance to protect your package due to unforeseen circumstances from third party shipping carriers, loss, or theft. 
• Forme does not hold or accept responsibility for packages that have been reported as delivered by the carrier and does not take responsibility for lost, misplaced, or incorrectly delivered shipments if the address information you have provided is incorrect or was incorrectly entered at the time of purchase. 
• You will receive tracking of your package and delivery notice.
• All international orders placed outside of the United States are final sale, and are not eligible for returns or exchanges. Custom/duty payments are the responsbility of the customer. This also includes military address orders to APO, FPO, DPO, etc.
