Returns
Return Policy
• Orders placed with the formescience15, Labor Day Sale, November Sale, Black Friday Sale, Cyber Monday Sale, and other holiday codes are only eligible for an exchange or refund towards store credit.
• Forme will happily accept returns/exchanges for eligible products within 21 days from the date of purchase.
• If you are attempting to return or exchange merchandise that is more than 21 days old, without tags, or damaged due to usage, unfortunately we will not be able to process the exchange or return per our policy.
• Certain discounted orders are only eligible for exchanges or store credit. See Sales and Promotions section below for more information.
• You will receive instructions and a label for the return/exchange process via the return/exchange portal under the return and exchange initiation section below. If you do not use our return shipping labels, we will not issue a refund. The provided labels are how our system tracks and verifies exchanges and returns for refunds.
• All exchange and return items must be in the mail within two weeks of the label being generated.
• The following products are final sale, and are not eligible for returns or exchanges: Ace Polo, men's Ace V-Neck, Arch Booster Sock, Contour Short, and Pro Short.
• Backordered items are subject to special terms and conditions. Please email us at hello@forme.science for more information.
• All merchandise must be returned in new condition with all tags and packaging intact. Please thoroughly evaluate your items before initiating the return process. Make sure they meet all the following mandatory requirements:
+ Unwashed
+ Tags still attached
+ Care label and size label intact
+ No deodorant marks
+ No makeup marks
+ No stains
+ No odors (including perfume)
+ No alterations
+ No holes
+ No tears
+ No other damage
• $15 will be deducted from your refund amount to cover the cost of shipping and handling. The original shipping costs and Route Protection charges are non-refundable.
• Exchanges have free shipping/fulfillment. Only one exchange can be made per original order.
• Defective products must be reported within 48 hours of delivery to qualify for a return or exchange. Please include photos of the damage in the report.
• Orders that have already been placed are not eligible for price adjustments.
• Please add Route Protection to your order. Route Protection is insurance for lost, stolen, or damaged packages.Forme is not responsible for lost or stolen packages and for any packages after they have been shipped. There will be no exceptions on lost or stolen items for replacements or refunds that do not have Route Protection. We highly recommend adding Route Protection to your order.

Return and Exchange Initiation
To initiate a return or exchange, please make sure your products meet the mandatory requirements, and start the process at this return/exchange portal. Please be prepared to provide the following information:
• Order number and zip code associated with the order
• Reason for return or exchange
• Required pictures of the products (please turn the product inside-out):
+ Front
+ Back
+ Care tag and size tag
•Please make sure the pictures are taken in adequate lighting and are in focus. Pictures must be sent in a .jpeg, .pdf, .png, or zip file.
Refunds
• Depending on your banking institution, refunds to your original payment method can take up to 5-14 business days after the product(s) are delivered back to Forme using our provided shipping label(s).
• Certain discounted orders are only eligible for exchanges or store credit. See Sales and Promotions section below for more information.
• Once a refund has been processed on our end, all future questions should go to your payment provider. Even if your card number has changed, your payment provider will ensure that it goes into the correct account.
• Shipping, handling, and Route Package Protection charges are non-refundable. Refunds for returned items will be issued in either original form of payment or store credit.
• $15 will be deducted from your refund amount to cover the cost of shipping and handling. The original shipping costs and Route Protection charges are non-refundable.
• Store credit information will be emailed to the email address associated with the order. Please make sure to check all folders including spam.
• Store credit does not expire. If you experience any issues, please email us at hello@forme.science.
Shipping
Due to carrier issues, some shipments may be delayed. We apologize for the inconvenience.
• All orders placed on Monday-Friday by 12 pm PST will ship within 1-3 business days, unless the item is backordered. Please check the product description to see if the item you are purchasing is on backorder.
• Orders placed during holidays, special promotions (Black Friday/Cyber Monday), and weekends may take an extra 1-2 business days for order processing.
• Delivery times will vary based on your delivery address. Most domestic orders will arrive within 3-7 business days.
• Please add Route Protection to your order. Route Protection is insurance for lost, stolen, or damaged packages.Forme is not responsible for lost or stolen packages and for any packages after they have been shipped. There will be no exceptions on lost or stolen items for replacements or refunds that do not have Route Protection. We highly recommend adding Route Protection to your order.
• Route claims for packages presumed to be lost by the carrier, where the status is not “delivered” must be filed no sooner than 7 days after the scheduled delivery date for domestic shipments, but not later than 15 days from the last checkpoint.
•Please double check your shipping address. Forme will not refund or replace packages that have the incorrect shipping address or unit/apartment/suite/etc number that is missing.
• Flat rate shipping refers to our flat rate shipping cost, which is a fixed cost for all customers. We currently do not use USPS Priority Flat Rate shipping.
Sales and Promotions
• Sales, discounts, reward codes all other promotions can not be combined. Only one discount can be used per order.
• Orders that have already been placed prior or post-sale are not eligible for price adjustments.
• Orders placed using the November Sale, Mother's Day Sale, July Sale, Memorial Day Sale, or Labor Day Sale discounts are only eligible for refunds toward store credit or exchanges.
International Returns
• All sales final for products shipped outside of domestic U.S. including Puerto Rico.
